Sickle Cell Crisis Plan Template

Fill in your own sickle cell crisis plan and download it as a PDF to keep on your phone, print, or share with the people who care for you.

Why a written crisis plan helps

A crisis is a hard time to explain your history, your usual medicines and what works for you. US guidance recommends setting up a pain action plan with your healthcare provider ahead of time so you are prepared when a crisis hits (NHLBI). A written plan also helps the people around you act quickly, and gives emergency staff the key facts at a glance.

How to use this template

  1. Fill in what you know now. Leave blanks rather than guessing doses.
  2. Take the PDF to your next haematology appointment and ask your team to check the medical sections.
  3. Save it on your phone, print a copy for your bag, and share it with your emergency contacts.
  4. Review it at least once a year, or whenever your treatment changes.
